This is Moda.

Moda is an icon pack which takes on an avant-garde styling with a squircle base split in two by a curved faded gradient to give each icon a unique vivid colour and identity.

The app contains:
Over 1250 icons @ xxxhdpi 192x192px
Over 24 cloud-based wallpapers to match
Material Design dashboard app (w/ request tool)
Supports all main launchers (nova/apex/action/go etc.)
Donation - doesn’t unlock any features purely a way to support me.
